Admissions Coordinator- Day Services/PHP/SOAP

Position Summary

Responsible for coordinating the intake and referral process for all clients admitted to Day Services Program (SOAP, PHP, Day Treatment) and supporting the Program Director with daily functions of the program including leadership responsibility.

Education & Licensing Requirements

- LCSW/ LICSW, LMHC, or LMFT license preferred, but not required
-Master’s Degree in a License Eligible Field
-Acceptable Degrees:
- Mental Health Counselor: LMHC or LMFT 
- Expressive Therapist: LMHC 
- Social Worker: LCSW or LICSW 
- Addictions Counselor: LADC1
-At least two years working with targeted population
-Comfortable with in-home therapy on an as-needed basis
-Own a vehicle with a valid drivers license
-Ability to pass a background and drug screening
-Experience with 1:1 counseling in a therapy setting
-Group therapy experience a plus
-Ability to utilize a computer and electric medical record software

Duties & Responsibilities

- Manage all referrals made to the program
- Represent South Bay in the community in a professional manner
- Attend monthly collateral meetings to represent South Bay Day Services, as needed
- Provide South Bay tours with collaterals, families and or clients, as needed.
- Set up intake appointments at referring agency (hospital, state agency, etc.)
- Manage intake log and completing monthly report
- Provide crisis management within the program
- Adhere to all Ethical and Best Practices in a professional manner and have the ability to lead and take on leadership
responsibilities.
- Participate in supervision and in-service trainings as required
- Conduct in-house as well as community outreach sessions with clients and relevant collaterals to establish linkages with care
providers in order to ensure continuity of care
- Orient all new clients to our programs on first date of service.
- Complete opening packets on new client intakes, including treatment plans within 24 hours.
- Perform outreach to consumers who are at risk or in need of transportation to and from the program as needed
- Manage transportation requests for all clients, including renewals. Transport consumers to and from program as needed.
- Consult with the Program Director in program development and serve as a clinical liaison to collateral provider agencies
- Facilitate groups as needed maintaining 72% referral access goal monthly.
- Work with our UR department to complete Initial and or Concurrent authorization reviews
- Assist MD as needed
- Other duties as required
