Admissions Coordinator

Position Summary

The Admissions Coordinator will be part of our Partial Hospitalization Program and will coordinate the intake and referral process for all clients admitted to Day Programs. The Admissions Coordinator will also support the Program Director with daily functions of the program including leadership responsibility.

Education & Licensing Requirements

* Master's degree in social work, counseling psychology or related field required.

Duties & Responsibilities

* Manage all referrals made to the program * Set up intake appointments at referring agency (hospital, state agency, etc.) * Manage intake log and completing monthly report * Conduct in-house as well as community outreach sessions with clients and relevant collaterals to establish linkages with care providers in order to ensure continuity of care * Manage transportation requests for all clients, including renewals. * Transport consumers to and from program as needed. * Consult with the Program Director in program development and serve as a clinical liaison to collateral provider agencies * Provide crisis management within the program * Participate in supervision and in-service trainings as required * Attend monthly collateral meetings to represent South Bay Day Services, as needed * Complete opening packets on new client intakes, including treatment plans within 24 hours. * Facilitate groups as needed maintaining 72% referral access goal monthly. * Work with our UR department to complete Initial and or Concurrent authorization reviews * Other duties as required

* Ability to lead and take on leadership responsibilities * Excellent organization, multi-tasking, and customer service skills * PC literacy and experience with MS Office Applications
