Treatment Coordinator

Position Summary

The Treatment Coordinator will work as part of South Bay’s Partial Hospital Program and will be responsible for coordinating treatment for all clients admitted to the Day Treatment Programs. The Treatment Coordinator will lead psychotherapy groups, and participate in treatment planning and consult with members of our multidisciplinary team.

Education & Licensing Requirements

* Bachelor’s degree in a Human Services related field from an accredited educational institution with 2 years experience working in multidisciplinary mental health or * Associate's degree with 4 years experience working in multidisciplinary mental health

Duties & Responsibilities

* Lead psychotherapy groups, and participate in treatment planning and consult with members of our multidisciplinary team. * Engage clients in the Program through outreach visits, monitoring of individual schedules, and team and individual meetings to ensure appropriate treatment compliance. * Conduct in-house, as well as community outreach sessions with clients and relevant collaterals to establish linkages with care providers to ensure continuity of care. * Coordinate treatment for all clients admitted to the Day Treatment Program.
