Treatment Coordinator-Brockton

Position Summary

The Treatment Coordinator will work as part of South Bay's Partial Hospital Program and will be responsible for coordinating treatment for all clients admitted to the Day Treatment Programs. The Treatment Coordinator will design group curricula, lead psychotherapy groups, and participate in treatment planning and consult with members of our multidisciplinary team.

Education & Licensing Requirements

* Bachelors degree and 2 years of experience in MMH (Multidisciplinary Mental Health Setting). * Associates degree and 4 years of experience.

Duties & Responsibilities

* Engage clients in the Program by monitoring individual schedules, and team and individual meetings to ensure appropriate treatment compliance. * Conduct in-clinic sessions with clients and relevant collaterals to establish linkages with care providers to ensure continuity of care. * Coordinate treatment for all clients admitted to the Day Treatment Program. * Design group curricula, lead psychotherapy groups, and participate in treatment planning and consult with members of our multidisciplinary team.

Experience & Skills

PC literacy and experience with MS Office Applications Lifting: Less than 15 lbs. Stooping/bending: Seldom Crawling/climbing/kneeling: Seldom Reaching/pulling/pushing: Seldom
